Dear Parent/Guardian,
YMCA Childcare Services is required to supply information about Y programs in 2020 to fulfill the requirements of a grant from the City of Fort Wayne that subsidized some Y program expenses during that time. Because your child(ren) participated in a YMCA child care program in 2020 we need your help to gather some information. You can find the survey below. Please fill out the survey once for each of your children.
